Thank you for using the online application to apply for the participation of your medical institution in the MediSave and MediShield Life scheme.

Please choose the type of your participation and ensure that you have the following information ready. Please note that clinics applying for the Community Health Assist Scheme (“CHAS”) would need to be accredited for Chronic Disease Management Programme (“CDMP”) under the MediSave and MediShield Life Scheme. Please also note that the Public Health Preparedness Clinic (“PHPC”) Scheme is only applicable for GP clinics.

My clinic / institution would like to participate in the MediSave and MediShield Life Scheme and / or the MediSave for Chronic Disease Management Programme ("CDMP"), Community Health Assist Scheme (“CHAS”) and Public Health Preparedness Clinic Scheme (“PHPC”).
My clinic / institution is currently accredited for CDMP, and we would like to participate in the Community Health Assist Scheme (“CHAS”) and Public Health Preparedness Clinic Scheme (“PHPC”).
My clinic / institution is currently accredited for CDMP and CHAS, and we would like to participate in the Public Health Preparedness Clinic Scheme (“PHPC”).
My dental clinic would like to participate in the Community Health Assist Scheme (“CHAS”) - Dental.
My institution is not HCSA-licensable. We would like to participate in the MediSave and MediShield Life Scheme.

1. Clinic licence under the Private Hospitals and Medical Clinics Act (“PHMCA”) or Healthcare Services Act (“HCSA”)
2. Personal details of the Clinic Manager

4. Email address of the medical institution
5. Personal details of the personnel in charge of submitting MediSave claims via the MediClaim eService
